[CA] Modem drops connection ~10x/day for ~1min each

Attached modem status page. It shows Rx levels from 0 to -7.4 dBmV, +40.3 to 45.8 dB SNR, +46 to 47.8 dBmV Tx. I currently have a 3dB attenuator in line but I have tried from 0-9dB with no success. I have never seen signal levels that were much different from those (the Rx level seems to have drifted down ~6dB since I last looked). I get a lot of correctables and uncorrectables, but I have checked all the connections in my apartment and they're all finger tight. Around 1.5 years ago I got some help here including a Mediacom-provided modem and a service visit where he replaced the connection between the line to the outside of my apartment and my wall jack. I currently am back on my Zoom Model 5370. I added a fan just because it felt warm all the time. I have tried a NETGEAR CM400 and a ARRIS SURFboard SB6141 and the Mediacom modem whose name I don't recall. None of the above has had any affect on my disconnect issue, which has continued with only brief interruption for nearly 3 years. Any ideas?
